Von Eiskammer bis Vakuumlaufband: Eine Wunderkammer voller Möglichkeiten

Reha im Spitzensport – Vorstellung neuester Erkenntnisse auf dem Internationalen GOTS-Kongress der Sportmedizin im Juni in Berlin

Prof. Ingo Froböse ist Leiter des Institutes für Rehabilitation an der Deutschen Sporthochschule (DSHS) in Köln und einer der renommiertesten Sportwissenschaftler Deutschlands. Auf dem internationalen Sportmediziner-Kongress der GOTS in Berlin wird er über neue Verfahren in der Reha von Spitzensportlern reden und – dabei kräftig provozieren.

Keine Angst vor hohen Lasten – die neue Reha bei Verletzungen von Spitzensportlern

Froböse: „Bislang sind wir immer vorsichtig und schonend an die Rehabilitation unserer Athleten herangegangen. Es ging immer nur um die Sicherung des Operations-Erfolges. Aber: dann haben wir die Biologie des Sportlers nicht im Blick. Die Strukturen, wie Muskeln und Bindegewebe müssen tatsächlich schneller und stärker belastet werden. Nehmen wir die Muskelfaser-Typen. Davon gibt es zwei: die schnellen und die langsamen. Habe ich einen Muskelfaserriss/einen Muskelbündelriss, sind meist viele, von den schnellen Fasern kaputt. Diese Fasern müssen intensivst und relativ hoch belastet werden, um den Sportler überhaupt zu seiner alten Leistung zurück führen zu können.“

Auch das Zusammenspiel von Muskeltraining und Dehnung muss noch intensiver umgesetzt werden.

„Bindegewebe wächst nur in Zugrichtung. Also hohe Gewichte auf die Muskeln und das Ganze mit der Dehnung zusammen ausführen – wir brauchen exzentrische Belastungen! Das gilt übrigens auch für Breitensportler und erst recht in der Prävention, damit manche Verletzungen erst gar nicht entstehen“, so der Sport-Professor.

Die Prävention von Sportverletzungen steckt fast noch in den Kinderschuhen

Froböse: „Da haben wir noch gar nichts. Wichtig wäre zum Beispiel ein optimiertes Training mit dem Ziel, eine Reha von vornherein zu vermeiden. Zum Beispiel bei Fußballern: da brauchen wir noch mehr neuro-muskuläres Training. Das Zusammenspiel von Nerv und Muskel, gekoppelt mit Präzision unter Zeitdruck ist extrem wichtig. Versuchen Sie mal auf einer Linie in hoher Geschwindigkeit und dabei exakt entlang zu laufen.“

Die Reha von Verletzungen im Spitzensport ist in Deutschland dafür hoch professionell.

Schwer verletzt und trotzdem schnell wieder fit – wie machen die das?

Wir Hobby-Sportler reiben uns oft verwundert die Augen. Gerade noch wurde unser Idol auf der Trage vom Fußballrasen gebracht und musste sich einer schwierigen OP unterziehen. Doch nach 8-12 Wochen sehen wir die Stars oft schon wieder im Spiel, während wir immer noch an einer kleinen Zerrung zu knabbern haben. Wie geht das?

4-6 Stunden Reha am Tag und eine „Wunderkammer“ voller Möglichkeiten

Prof. Froböse gibt einen Einblick in die Welt der Spitzensport-Medizin: „Im Leistungssport haben wir allein schon eine höhere Dichte in der Reha. Unsere Athleten haben 4 bis 6 Stunden Reha – jeden Tag! Ein Breitensportler bekommt 1-2 mal pro Woche Physiotherapie, das ist absurd. Da komme ich nicht an die alte Leistung heran.“ Dazu kommt die viel höhere Qualität. Viele Sportler bekommen relativ schnell nach der OP zuerst eine Unterwasser-Therapie. Dann gibt es die sogenannten Vakuum-Therapie – Laufbänder, auf denen der Athlet, fast wie in einer Schwerelosigkeit, geht. Die Beine und der Oberkörper sind dabei quasi getrennt, der Oberkörper schaut aus dem Vakuum heraus und schon kann er „schwerelos“ joggen, ohne irgendwelche Strukturen zu belasten. Froböse: „Wichtig ist auch unsere Eiskammer. In Shorts, mit Schuhen, Handschuhen und Mütze gehen die Sportler bei unter minus 100 Grad Celsius(!) da hinein. Die Kälte wirkt gegen Entzündungen und setzt physiologische Reize. Neben den normalen Sportphysiotherapien haben wir natürlich auch modernste Trainingsgeräte, arbeiten mit Vibration, Elektrostimulation und vielem mehr. Da gibt’s eine ganze Wunderkammer an Möglichkeiten.“

Das Wichtige ist, so Froböse, dass der Sportler wirklich schnell an seine Leistung wieder anschließt. Ein „bisschen“ reicht da nicht. Schließlich darf zum Beispiel ein Fußball-Spieler nach einer Sportverletzung nicht statt 50 Millionen Euro plötzlich nur noch 20 Millionen Euro wert sein.

Doch obwohl viele Sportler gleich gut in ihren Leistungen sind, bringen sie häufig völlig unterschiedliche biologische, körperliche Voraussetzungen mit. Prof. Froböse: „Bei Sportlern mit schwachem Bindegewebe zum Beispiel, müssen wir vorsichtiger sein. Da dürfen wir nicht zu früh beginnen.“
