GOTS-SIGASCOT Fellowship Italien 2019 | Reisebericht

Bergamo – Milano – Florenz – Parma – Rom

Zwei intensive Wochen im November im wunderschönen Italien inklusive fünf Stationen mit wunderbaren Begegnungen, wertvollen Erkenntnissen rund um die rekonstruktive Chirurgie des Bewegungsapparates und sehr viel, sehr gutem Essen liegen nun hinter uns.

Da wir uns beide noch zu Beginn unserer Spezialisierung befinden, kam diese Art von Travelling Fellowship für uns genau zur richtigen Zeit. Wir durften von hervorragenden Lehrern neue Sichtweisen, Techniken und Perspektiven lernen, die uns für unseren weiteren Weg sehr hilfreich sein werden.

Bereits vor unserer Reise erhielten wir einen detaillierten Zeitplan, der unsere Neugier und die Lust auf mehr weckte. Unser Abenteuer begann an einem regnerischen Sonntagabend in Bergamo, wo wir von Francesca de Caro begrüßt und zu einem ersten (von zahlreichen) hervorragenden Abendessen eingeladen wurden. Bei einem guten Glas Wein konnten wir uns kennenlernen und erste Erfahrungen austauschen. Der folgende Morgen begann bereits um 7 Uhr in der privaten Humanitas Klinik in Bergamo, in der Vincenco Madonna und Vincenco Condello nebst dreier weiterer Fachärzte und einem Assistenzarzt den größten Teil der elektiven Orthopädie abdecken. So durften wir an zwei Tagen zahlreiche spannende Operationen miterleben- von der Revisions-Kreuzbandplastik über bilaterale Knie Totalendoprothesen bis hin zu Umstellungsosteotomien und der „Lipogem“ Therapie, in der aus autologem Abdominalfett Stammzellen gewonnen und ins Kniegelenk injiziert wird. Da wir beide unseren Interessensschwerpunkt im Bereich der Kniegelenkschirurgie gelegt haben, traf dieses Programm genau unsere Wünsche und wir konnten bereits gleich zu Beginn spannende Erkenntnisse gewinnen. Der erste Tag endete mit einer kurzen Scientific Session, an der wir eigene Projekte aus den Unispitälern Basel und Wien unseren Gastgebern präsentieren und diskutieren durften, gefolgt von einem ausgezeichneten Abendessen und gutem Rotwein. Dieser Tagesablauf (OP-Präsentationen-Abendessen) wurde uns alsbald zur vertrauten Routine während dieses Fellowships.

Nach einer Vormittagssession im OP der Humanitas Klinik hatten wir am zweiten und letzten Tag in Bergamo noch die Möglichkeit bei nasskaltem Wetter die wunderschöne Altstadt Bergamos zu erkunden, bevor es am Abend dann im Regionalzug nach Mailand weiterging.

In Mailand angekommen wurden wir am Bahnhof freundlich empfangen um anschließend in den Genuss eines großartigen Essens mit typischen Mailänder Köstlichkeiten mit Paolo Arrigoni und seinem Team zu kommen. Paolo Arrigoni erwies sich als sehr zuvorkommender Gastgeber, der uns am Hotel abholte und mit der Metro direkt ins Gaetano Pini Institut begleitete. Dort erlebten wir sehr spannende arthroskopische Operationen an Schulter und Ellenbogen, bevor am Nachmittag eine sehr gut organisierte Scientific Session mit Dr. Arrigoni und Dr. Berruto und deren Teams folgte. Vom Thema Knie über Schulter und Ellenbogen erfolgte in angenehmer, kollegialer Atmosphäre (versüßt durch die mitgebrachte Schweizer und Österreichische Schokolade) ein angeregter Wissensaustausch. Nach einem erneut sehr gelungenen Abendessen mit gutem Chianti mit Dr. Berruto und seinem Team durften wir den Meister am nächsten Tag im OP bei einer Knie-Totalprothese, einer Unikondylären Prothese sowie einer Subchondroplastie zur Behandlung von Knochenmarködemen, begleiten. Gestärkt durch einen kleinen Snack in einem der ältesten und bekanntesten Panini Ristorantes in Mailand ging es anschließend mit vielen neuen Eindrücken weiter nach Florenz. In dieser Geschichtsträchtigen und wunderschönen Stadt, die uns nun endlich mit freundlicherem (Italien-typischem Wetter) begrüßte, hatten wir dann auch die Gelegenheit unsere Koffer einmal auszupacken, da wir hier insgesamt 4 Tage verbringen durften. Am ersten Abend durften wir Nives Sagramola von der SIGASCOT kennenlernen, die für uns perfekt alles organisiert hatte. Mit ihr hatten wir ein vorzügliches Abendessen in einem wunderschönen kleinen Ristorante zusammen mit Dr. Francesco Giron und seinem Team. Auch wurden wir zu diesem Anlass mit der offiziellen Krawatte der SIGASCOT beschenkt, die wir mit Stolz in Ehren halten.

Es folgte ein aus operativer Sicht höchst spannender und lehrreicher Tag im Universitätsklinikum Careggi mit Dr. Giron und Dr. Matassi. Neben einer Multiligament Rekonstruktion (VKB, HKB und LaPrade PLC Rekonstruktion) folgten weitere primäre VKB Rekonstruktionen sowie eine Tuberositas Osteotomie mit anschliessender MPFL Plastik. Noch beeindruckt von den Operationen folgte am Abend eines der kulinarischen Highlights mit Dr. Matassi- das Original Florentinische Bistecca – ein wunderbar zart gegrilltes (riesengroßes) Porterhouse Steak, von dem uns noch heute das Wasser im Munde zusammenläuft.

Am Samstag folgte um 10 Uhr eine sehr interessante Scientific Session, bevor Dr. Matassi und Dr. Losco uns an ihrem freien Tag „Ihr Florenz“ zeigten- ein besonderes und einmaliges Erlebnis inklusive privater Führung durch die Kathedrale und das begleitende Museum. Willkommen war uns dann aber auch der folgende Sonntag, der zur freien Verfügung stand. Einmal in Florenz ließen wir es uns selbstverständlich nicht nehmen die Uffizien, Davids Michelangelo und die Schönheit der Stadt zu erkunden, was sich durch den zeitgleich stattfindenden Florenz Marathon etwas erschwert gestaltete. Hier übrigens in Florenz, auf eigene Initiative hin, war es auch, dass wir zum einzigen Mal eine typisch italienische Pizza verspeisen sollten.

Am Montag verließen wir Florenz mit vielen positiven Eindrücken um am Abend in Parma unseren nächsten Gastgeber Paolo Adravanti kennenlernen zu dürfen. Nach einer weiteren Präsentation unserer Forschung folgte ein erstes für die Region typisches Abendessen – die Ankündigung des hervorragenden Essens der Emilia Romagna waren absolut nicht übertrieben. Beindruckt waren wir am Folgetag, dass Prof. Adravanti und sein Team an einem Tag insgesamt 11 Operationen in der Privatklinik „Città di Parma“ bewerkstelligten. Wir durften beiwohnen, mit welcher Akribie Prof. Adravanti nebst Endoprothesen, VKB Plastiken und Revisionseingriffe durchführte. Sicherlich eines der Highlights im Hinblick auf die gesehenen Operationen stellte eine chronische Patellarsehnenruptur nach Patellafraktur dar, welche mittels Achillessehnen Allograft aufwendig rekonstruiert wurde. Der Operationstag endete gegen 20.30 Uhr was uns –an Mitteleuropäische Verhältnisse gewöhnt- erst sehr spät in den Genuss von Anolini, Culatello und Casoncelli kommen ließ. Nach einer herzlichen Verabschiedung durch Prof. Adravanti stand der folgende Tag ganz im Zeichen der Scuderia Ferrari- Dr. Dini führte uns ins Ferrari Museum nach Maranello wo sich Kindheitsträume verwirklichten.

Den würdigen Abschluss unseres „Giro d`Italia“ stellte die ewige Stadt dar! Rom empfing uns mit strahlendem Sonnenschein und mit erneut sehr herzlichen Gastgebern. Am Donnerstag durften wir im Campus Bio-Medico in Rom Prof. Franceschi im OP folgen, wie er nebst Knie Prothesen auch eine VKB Plastik, Computer-navigierte Schulterendoprothesen sowie Schulter und Kniegelenksarthroskopien absolvierte und uns nach der letzten Scientific Session anschließend in ein Restaurant führte, in dem wir die besten Spaghetti Carbonara unseres Lebens genießen durften.

Unsere letzte Station stellte der Besuch in der Concordia Privatklinik bei Giovanni di Giacomo dar, der als ATP Arzt bekannte Sportler Persönlichkeiten wie Roger Federer, Rafael Nadal und Andre Agassi in seiner Funktion als Turnierarzt betreute und sich auf Schulter und Ellenbogen spezialisiert hat. Wir durften ihn bei der klinischen und radiologischen Diagnosefindung in der Sprechstunde begleiten und konnten hierbei einige interessante Techniken und Tipps lernen. Am Ende des spannenden Tages erhielten wir ein von ihm verfasstes Lehrbuch zur funktionellen Schulteranatomie als Präsent (siehe Foto). Auch dieser Tag sollte mit einem köstlichen Abendessen zu Ende gehen, bevor früh am Samstagmorgen unsere gemeinsame Zeit dieses außergewöhnlichen Fellowships ein Ende fand und wir nach Hause zurückkehrten.

Zum Abschluss möchten wir uns recht herzlich bedanken: bei PD Dr. Ezechieli und der GOTS, der SIGASCOT und Nives Sagramola für die perfekte Organisation und die finanzielle Unterstützung wie auch bei unseren wundervollen Gastgebern- Vincenzo Madonna und Vincenzo Condello in Bergamo, Paolo Arrigoni und Massimo Berruto in Mailand, Francesco Giron und Fabrizio Matassi in Florenz, Paolo Adravanti in Parma sowie Francesco Franceschi und Giovanno di Giacomo in Rom. Jede einzelne dieser Begegnungen hat uns fachlich wie auch persönlich bereichert und wird uns in guter Erinnerung bleiben – wir bedanken uns bei allen Gastgebern für die Gastfreundschaft und die Bereitschaft, ihr langjährig erworbenes Fachwissen mit uns zu teilen.

Die Nachwuchsförderung legt den Grundstein für die Patientenversorgung kommender Generationen. Umso wichtiger ist der intensive Austausch und die Möglichkeit sich ständig weiterzubilden und zu lernen- entsprechend ist es nicht hoch genug einzuschätzen, dass die GOTS und SIGASCOT uns und weiteren Generationen junger Kollegen diese Erfahrungen ermöglichen. Unsere Erwartungen wurden vielfach übertroffen, so dass es uns ein Anliegen ist, jungen, motivierten Kollegen dieses Fellowship wärmstens ans Herz zu legen.
